Knee Pain Needs A Whole-Chain Assessment

Knee pain can come from a range of structures and can behave very differently from one person to another. Some people mainly notice it when the knee is loaded; others have pain at rest, during sleep or after activity.

When assessing knee pain treatment in Brisbane, I want to understand exactly when the symptoms occur and then look at the knee in the context of the whole lower limb.

The Hip, Foot And Ankle Can Change Knee Load

The knee sits between the hip and the foot, so what happens above and below it can influence the load it has to manage.

The hip may be weak, restricted or poorly controlled. The foot and ankle may lack stability or movement capacity. These factors do not automatically 'cause' knee pain, but they can contribute to the way load is distributed through the knee.

That is why I do not want to assess only the spot that hurts.

How I Treat Knee Pain

Where appropriate, I may use shockwave therapy on painful or sensitive tissues around the knee, alongside deep tissue work, dry needling and other hands-on techniques.

The purpose of this initial treatment is often to reduce pain enough that we can begin loading the leg more comfortably.

From there, exercise becomes important. We may need to improve hip strength, control through the knee, calf and ankle capacity, foot stability or general lower-limb strength.

The exact program depends on what the assessment shows. The goal is to improve how the entire leg shares load rather than expecting the knee to tolerate everything by itself.

What To Expect

Four to six sessions is a useful general guide for many musculoskeletal pain conditions, but knee pain varies significantly depending on the tissue involved, how long it has been present and the loads you need to return to.

Your progress is reassessed throughout treatment, with the balance shifting from symptom reduction toward strength and capacity as appropriate.

Frequently Asked Questions

Why are you assessing my hip when my knee hurts?
Because hip strength and control can influence the forces reaching the knee. It is one potential contributor I assess rather than an assumed cause.

Can shockwave therapy be used for knee pain?
It can be used for selected soft-tissue or tendon-related presentations. It is not appropriate for every type of knee pain, so assessment comes first.

Will I need strengthening exercises?
Often, yes. When reduced strength or load tolerance is relevant, progressive exercise is usually an important part of improving the knee's capacity.
